Thanks for your submission! However, your application is not considered complete and will not be reviewed by our Financial Assistance Committee until you email the following items to info@demarionsf.org (as outlined in the Terms and Conditions):
 
1.       Copy of driver’s license or proof of identification.
2.       Copy of lease or mortgage document (if rent/mortgage loan assistance is being requested).
3.       Copy of bill(s), if applicable (ex. notice of failure to pay rent, utilities bill, etc.).
